Terms and Conditions for Specimen Collection Services
CMT Mobile Testing Services Results Lab

1. Scope of Services
CMT Mobile Testing Services Results Lab (“Company”) provides professional specimen collection services for drug and alcohol testing, lab testing, and other related medical screenings. Services may be conducted on-site, at client locations, or at designated collection sites.
2. Client Responsibilities
  • The Client must provide accurate information regarding individuals to be tested, collection locations, and testing requirements.
  • The Client is responsible for ensuring a safe, accessible environment for specimen collection.
  • The Client agrees to comply with all applicable federal, state, and local regulations related to specimen collection and testing.
3. Chain of Custody
All specimens will be collected following strict chain-of-custody procedures to maintain integrity and traceability. Any tampering, refusal, or irregularity during collection will be documented and reported to the Client.
4. Confidentiality
All personal and medical information obtained during the collection process will remain confidential and handled in compliance with HIPAA and other applicable privacy laws. Test results will only be released to authorized parties as designated by the Client.
5. Fees and Payment
  • Payment terms are [insert terms: e.g., due upon receipt, Net 15, etc.].
  • Additional fees may apply for after-hours service, travel beyond designated service areas, or missed appointments.

6. Cancellations and Rescheduling

8. Liability and Indemnification
While every precaution is taken to ensure accuracy and professionalism, the Company is not liable for results interpretation, laboratory processing errors, or decisions made based on test results.
The Client agrees to indemnify and hold harmless CMT Mobile Testing Services Results Lab, its employees, and agents from any claims arising from the use or misuse of test results.
9. Compliance
All collections are performed in accordance with applicable DOT, non-DOT, or workplace policy standards, depending on the client’s request and regulatory requirements.
10. Termination
Either party may terminate this agreement with written notice. The Client is responsible for payment of all services rendered prior to termination.
11. Governing Law
This agreement shall be governed by and construed in accordance with the laws of the State of [Insert State].
12. Acknowledgment
By scheduling or requesting specimen collection services, the Client acknowledges and agrees to these Terms and Conditions.
